A photocopy of the insurance card providing coverage for the student must be provided with the physical, concussion acknowledgement, and proof of insurance forms.  The physical must have been completed within the last year and be valid for the entire sport season.  A physical must be on file for the student to participate in tryouts, practices, and events. 

JH Athletic Fee (8th Grade):  $40 per sport/maximum of $80 per student

HS Athletic Fee (9th-12th):  $40 per per sport/maximum of $80 per student
